It depends on the vendor & context, difficult to generalize. The true north is option #1 but it is important for both parties to work in the direction and create an environment of trust. It important to acknowledge that in our kind of work it is not possible to attribute every hour to an outcome at an unit level. Hence I focus on being 'directionally correct' by the hour i.e is the billable hours being spent on doing work that will take us to the desired outcome.

I think that the relation with the 3rd party or the firm will dictate the comfort you may have about the hours you got charged.

When it gets more difficult is when a consultant requires help from 'juniors' or supporting people from the firm.   Sometimes, you don't get all the value you pay for but sometimes, you get amazing results.

It's really a matter of trust.
