Dropping my AI/ML consultant even though I knew it was going to set my project back 3+ months. He had brought a lot of good work forward but decided to play games with his firm and force a substantial hourly increase. He stopped showing up for work when that tactic didn't work. We were having none of that and pursued hiring a full stack contractor which has worked in the team's favor.

That was a tough call, but a solid one. Most people wouldn’t have the guts to make that type of decision, especially knowing it would set the project back. I respect that. Cutting ties early with someone unreliable usually saves more time than it costs.
Cut loose a hire that lied in the background check that human resources missed. It was a snowball that kept growing that human resources just shouldn't have let even through the first phase and then caused many months of disruption to cut loose.
